Under Texas Board of Chiropractic Examiners Rule §76.2, a patient may request that patient records be disclosed to the patient or another person.
A written request should include:
- The specific information or records to be disclosed.
- The person to whom the records should be disclosed.
- The signature of the patient or other person legally authorized to act on the patient's behalf.
Patient records will be disclosed, after receipt of any applicable fees, within 15 business days from the date of the request unless the request is denied as permitted by Rule §76.2.
